Are you a compassionate and experienced nurse with a passion for providing outstanding care within the community? We are seeking a dedicated Nurse Care Manager to oversee and enhance the delivery of high-quality care for individuals with complex needs, including dementia.
This is an excellent opportunity offering 4 day working week, hybrid working, flexible hours, opportunity to not deliver personal care and so much more.
Key Responsibilities of a Nurse Care Manager:
• Conduct assessments to ensure tailored, person-centred care plans.
• Mentor and train care staff to maintain clinical and professional standards.
• Act as a key clinical resource and provide expert advice to care teams.
• Support families navigating complex care needs, with a focus on dementia care.
• Develop and implement effective care planning and risk management tools.
• Foster a culture of excellence and continuous improvement.
Nurse Care Manager Requirements:
• Registered Nurse (Adult, Mental Health, or Learning Disability) with current NMC registration.
• Significant clinical experience.
• Expertise in mentoring, training, and supporting care teams.
• Strong leadership and communication skills.
• A passion for community-based care and empowering others.
• You must be a driver with valid driving licence and access to a vehicle.
Benefits:
